All Hereinafter referred to as “the Model”

and

John A Miller, PO Box 13693, Tampa, FL 33614,  813-495-1981

All Hereinafter referred to as “The Photographer”

For valuable consideration received, I hereby grant to John A Miller of Photo Enrichments, the unrestricted and irrevocable right and permission with respect to the photographs and/or film (“the Work”) John A Miller has taken of me or in which I may be included with others to

  1. Copyright the same in his name or any other name he may select.
  2. Use, reuse, publish, and republish the same, whole or in part, separately or in conjunction with other photographs in any and all media now and hereafter known for illustration, art, promotion, advertising, trade, editorial, or any other purpose whatsoever, without restriction as to alteration.
  3. Use my name in conjunction therewith if he so decides.

I hereby release and discharge John A Miller of Photo Enrichments from any and all claims and demands arising out of or in connection with the use of the photographs, including any and all claims for libel or invasion of privacy.

This authorization and release shall also insure to the benefit of the heirs, legal representatives, licensees, and assigns of John A Miller, as well as the person(s) for whom he took the photographs.

I hereby affirm that I am of full age and have the right to contract in my own name.  I have read the foregoing prior to its execution, and I fully understand the contents thereof.  This agreement shall be binding upon me and my heirs, legal representatives and assigns.

Permission herein granted is absolute and final and shall not be subject to further inspection or approval by the Model at any stage in the use of the Work

Use of the Work may be in conjunction with the Model’s own or fictious names.

Photo Enrichments shall own all rights in the Work which shall accrue to the benefit of his/her successors, legal representatives and assigns.

TRADE-FOR-PHOTO (TFP)…. There is no form of payment being exchanged between John A Miller of Photo Enrichments and/or Model.  Whether TFP or PAID shoot, Photos may NOT be altered and/or sold by the Model.  Photos will be used by the Photographer’s discretion, but not limited to his social media platforms or forums such as Instagram, Facebook, Twitter, Model Mayhem, Company website, OnlyFans, Pateron, or any other platforms present or future.

A certain number of photographs will be given to the Model depending on the length of the shoot.  Photographs will be provided to the Model in a pixel grade suitable for use on Social Media Platforms or the Model’s website.  The Model may use these photographs as they wish.  The Model must credit John A Miller of Photo Enrichments when using/posting any photos from this shoot.  If the model wishes to us these photographs for commercial or media purposes, permission must be given by John A Miller of Photo Enrichments.

The name of the Model will be used and tagged unless the Model requests otherwise.

The Model agrees that the provisions contained herein shall be binding upon the Model and his/her successors, legal representatives and assigns.  The parties agree that any or all of this agreement may be submitted to the other party in person or in legible and recordable electronic form and upon acknowledgement of receipt by the receiving party shall become valid parts of the agreement.

THIS LEGAL AGREEMENT APPLIES TO ANYTIME THE MODEL IS PHOTOGRAPHED BY JOHN A MILLER OF PHOTO ENRICHEMENTS AFTER THE DATE SIGNED.
